Patients with time sensitive conditions have significantly better outcomes at lower total cost when care is pro-actively planned across the many agencies and institutions that are typically involved in an episode of care for any of these conditions. This toolkit is comprised of resources aimed to assist in their development and improvement of multi-organizational systems of care coalitions.

This toolkit  is intended to support local public health agencies, regulatory agencies, hospitals, emergency medical services (EMS) regulatory and provider agencies, payers, senior appointed or elected officials, and others champions wanting to improve systems of care for time-sensitive conditions.
